You need to make an installation floppy prior to installation.
This can be done in one of two ways.

1) From MS-DOS or WinXX (safest):

Place a floppy in your floppy drive.
Run 'BOOTMENU.EXE A:'
Leave floppy in drive and reset.

2) From Linux / FREE-BSD:

Place a floppy in your floppy drive.
Run 'cat BOOTMENU.BIN > /dev/fd0H1440'
Leave floppy in drive and reset.
